Need to populate field values of the tabe to the Process Automation Questions

Hi all,

 

The requirement is to populate the incidents' field values to the Process Automation Questions asked as below screenshot.

So, here I need to populate the Assignment group's EMAIL to the Email Address question (should be made read only) and the Configuration Item name to Circuit ID (should be made editable).

 

I've used a decision tree to run the Process Automation and is done via the Playbook for incident task.
