Script Include error

I have marked this script include "PopulateOnsiteFacilityLocation" as INACTIVE, then also I see this error. Can anyone tell how can this error be solved.

Hey,
seems as if you are in the wrong application scope. You will need to switch into the application scope of the script include. To learn more about Application Scopes, check this out.
On another topic: Setting script includes to inactive should only be done as an alternative to delete them (e.g. retiring functionality from you instance).

If you have deactivated the script inlcude, it should not pop, but can you check if your script is still trying to access this script inlcude in someway.
Try clearing the cache, might help
Type "cache.do" in the app nav and press enter.
