Associates forms together

Please examine the following scenario and let me know how to resolve the issue in the last point

Users fill a form on the service portal and submits
A task is created, and a new select box field (with 5 options) is in the task for agent to choose an option
Based on the option chosen by the agent, a notification is sent to the user
In the notification, there is a link to another form on the service portal that the user needs to fill

The issue i am trying to solve is how to associate the 2 form together so that so that there will be a way to go back and forth between the two forms.

Hi @DebbyDebby ,

Both of the form needs to be mapped together either by some reference field in any one of the table or by some other condition by which we can able to identify the correct associated forms.
