Pre-populate change request record with data from Case, when redirected through interceptor page

Hi community,

I have a requirement to prepopulate requested_by on Change Request with contact from the Case record, but the process should be as follows:

on Case record: UI action(link) Create Change -> Change Request interceptor page with available change models -> user chooses a model and creates a new Change Request -> there is a contact from the Case record in the field requested_by.

Does anyone have an idea how to achieve this?
