Approve/Reject via email is not linking back to ServiceNow ticket

I received an email from ServiceNow to approve / reject an access request. When I click on "Approve" or "Reject", a new browser is opened on a new tab but it doesn't link back to ServiceNow.
In the past, when I click on "Approve" or "Reject", a pop up would open asking for confirmation and I click ok, it will update the ServiceNow ticket.
The functionality was working before. I don't know if I entered something incorrectly and broke it. Please provide guidance as to how I can fix this.
