Use the Legal hold notification intake form to submit the legal hold notification matter.

| Short description | Brief overview of the legal hold purpose. |
| Summary of legal matter | Detailed summary of the legal issue. |
| Lawsuit filed by (party name) | Name of the party who filed the lawsuit. |
| When was the legal hold issued? | Date of the legal hold issued. |
| Legal hold start date | Date of the legal hold start date. |
| Legal hold end date | Date of the legal hold end date (if available). |
| Notice template | Legal hold notice template. An editable legal hold notice template is provided with the Legal Hold Notification application. |
| Custodial data | The legal user responsible for preserving data related to the legal hold.
