# Health and Safety action form {#ariaid-title1}

Safety agents can use the Health and Safety action form to assign an action for a safety incident or observation.

| Field | Description |
|-|-|
| Health and Safety task | Parent Health and Safety record this action is related to. If the action belongs to an existing safety record such as an injury or risk assessment, select that record. |
| State | State of the action. |
| Number | System-generated unique number for the action. |
| Assignment group | User group responsible for this action. |
| Assigned to | User responsible for this action. |
| Location | Location this action item applies to. |
| Due date | Date by when this action should be complete. |
| Type | Type of the action. * Corrective: An action to eliminate the causes of non-conformities or undesirable situations to stop them from happening again. * Preventive: An action to prevent the occurrence of such non-conformities. * Task: A generic task assigned as an action. * Learning: An action that includes a learning for the user, for example, completing a training. Note: The Learning option in this field appears only when the Learning Core (sn_lc) application is installed on your instance.
