Use the Assessment Metric Type New Record form to create an attestation template to create the attestations. Use the template form to create a record.

Field	Description
Name	Name of the template.
Description	Information about the template.
Active	Option to enable the template.
State	State of the assessment. The available options are as follows: Draft Published This field is auto-filled.
Metric categories	Category that represents a theme for evaluating assessable records in a given metric type. It displays the name of the template, description of the category, its weight, order, and so on.
