## Summary of Maturity assessment questionnaires

Maturity assessment questionnaires are used within Impact maturity assessment accelerators to evaluate the maturity of processes or capabilities by collecting responses to domain-grouped questions.
These assessments can include multiple participants, whose combined responses generate maturity scores per domain and overall.
The results help identify gaps and guide improvements through a tailored action plan with specific recommendations.

## How Maturity Assessments Work

* Multiple participants can independently complete the questionnaire.
* Responses are aggregated to produce maturity scores for each domain and an overall score.
* Identified maturity gaps enable selection of target maturity levels for improvement.
* Accelerators generate customized action plans based on selected targets to close gaps.

## Question Types and Scoring

* Only single-select questions are used, each with statements describing increasing maturity levels.
* Answers must reflect current practices, not plans or documentation.
* All questions and domains are equally weighted in scoring, with no bias towards any particular item.

## User Interface Features and Workflow

* **Multi-Participant Configuration:** Add participants, assign roles and domains, generate and track surveys from a single interface.
* **External Surveys:** Participants receive surveys limited to their assigned domains, supporting target maturity specification.
* **Aggregated Results:** Assessment owners view combined participant responses in multiple visualization formats, while participants cannot see others' answers.
* **Progress Tracking:** Monitor participant completion status over the 14-day collection period.
* **Action Plan Management:** View, download, and customize prioritized action items by domain, including ownership and notes.

## Existing Assessments and Related Accelerators

Earlier assessment models, such as the Impact Staffing and Roles Review-On Demand and the Adopt AI Governance assessment, remain supported. Related maturity assessments include ITSM Maturity Assessment, Portfolio Governance Maturity Assessment, and Strategy Governance Maturity Assessment---each available on demand.
A maturity assessment questionnaire collects responses that an Impact maturity assessment accelerator scores, using questions grouped into domains.

## How maturity assessments work {#maturity-assessment-questionnaire__section_tlc_ccl_rkc}

Several Impact accelerators assess the maturity of a process or capability by asking a set of questions grouped into domains. A maturity assessment can involve multiple participants. The main user completes the
questionnaire and can also invite others in their organization to complete it independently. Responses across all completed questionnaires are aggregated to produce a maturity score for each domain and an overall maturity
score for the accelerator.{#maturity-assessment-questionnaire__maturity-assessment-questionnaire-para-1}

Using these results, you can identify maturity gaps across domains and select a target maturity level for each one you want to improve. Based on those selections, the accelerator
generates an action plan with a customized list of recommendations to close the gaps.{#maturity-assessment-questionnaire__maturity-assessment-questionnaire-para-2}
Note:
For the accelerators that use a maturity assessment questionnaire, see [Strategy Accelerators](https://www.servicenow.com/docs/PMESeUqJB90p5kV_bVPWDQ "Use Strategy Accelerators to go deeper on organizational and governance general guidelines with the ServiceNow platform and upskill your team to maintain business performance and drive success.").

## Question types {#maturity-assessment-questionnaire__section_ulc_ccl_rkc}

A maturity assessment questionnaire uses single-select questions only. For each question, you choose the one statement that best reflects current practice from a set of options
describing increasing levels of maturity. Your answer should reflect how work happens today, not documentation, goals, or planned improvements.{#maturity-assessment-questionnaire__maturity-assessment-questionnaire-para-3}

## Question and domain weighting {#maturity-assessment-questionnaire__section_vlc_ccl_rkc}

Every question and every domain carries the same weight when responses are scored. No question or domain counts for more than another.{#maturity-assessment-questionnaire__maturity-assessment-questionnaire-para-4}

## Existing assessments {#maturity-assessment-questionnaire__section_wlc_ccl_rkc}

Accelerators that were built on the earlier assessment model continue to work as before, including the Impact

## Maturity assessment workflow UI features {#maturity-assessment-questionnaire__section_workflow_ui}

For every maturity assessment on-demand accelerator, the following UI features are available:

* Multi-Participant Assessment Configuration:Add participants to an on-demand maturity assessment and assign each one a role and one or more domains. Generate surveys for your participants and track their delivery
  from a single configuration view.

* External Participant Surveys:Send participants external survey questionnaires. Each participant sees only the questions for the domains assigned to them. It also supports target specification during
  assessments.

* Aggregated Participant Results: View responses from all participants combined in the Review Results step of your Playbook. As the assessment owner, you see all aggregated data, while participants can't see each
  other's responses. You can also use the multiple visualization formats (KPI cards, bar charts, heat maps, tables) to review aggregated survey results. Users can monitor assessment progress over the 14-day survey
  collection period.

* Participant Completion Tracking:Monitor each participant's completion status and response date so you know whose input is still outstanding.

* Action Plan:
  * View and download the action plan page displaying prioritized items organized by domain.
  * Modify suggested owners and add notes to action items.
