  • The decision form enables you to create a decision or plan for a demand to resolve or mitigate a risk or issue.

    Table 1. Decision form
    Field Description
    Number System-generated ID number with a configurable prefix.
    State Current state of the decision. Decisions are created in the Open state.
    The available values are:
    • Pending
    • Open
    • Work in Progress
    • Closed Complete
    • Closed Incomplete
    • Closed Skipped
    Priority Urgency of approving or implementing the decision based on the possible impact. Decisions are created with Low priority.
    The available values are:
    • Critical
    • High
    • Moderate
    • Low
    • Planning
    Decision status Status of the decision. The available values are Pending, Approved, and Rejected.
    Impact Impact of the decision on the demand. Decisions are created with Low impact.
    The available values are:
    • High
    • Medium
    • Low
    Approval required Indicates whether approval of the decision is required.
    Estimated Cost Estimated cost of implementing the decision.
    Due date Requested date for decision approval or implementation.
    Assigned to Resource assigned to work on the decision.

    The default value is the name of the user creating the decision is populated.

    Parent Demand associated with the decision. This field is auto-populated.
    Short description Brief summary of the decision.
    Description Brief description of the decision and its impacts on the demand.
    Work notes Information about the decision.
