# Create a safety incident or observation as a safety agent or supervisor

# Create a safety incident or observation as a safety agent or supervisor {#ariaid-title1}

Begin the investigation for a safety incident or observation by reporting it on behalf of an employee.

## Antes de Iniciar

Role required: sn_ohs_im.operations_manager, sn_ohs_im.manager, or sn_ohs_im.agent

## Procedimento

1. Navigate to AllHealth and SafetyHealth and Safety Workspace.
2. Select the incident management icon (![Incident Management icon.]()).
3. In the Lists tab, open the safety incident or observation list.
   * For the safety incidents list, select Safety Incidents and then All.
   * For the safety observations list, select Safety Observations and then All.

4. Select New.
5. On the form, fill in the fields.
   * For a safety incident, fill in the fields in the Enter incident summary activity in the Report an incident tab.
   * For a safety observation, fill in the fields in the Details tab.

   Nota:  
   To enable the map component in the Health and Safety Workspace install the Geo map component \[sn_geo_map\] plugin. For more information, see [Additional features in Health and Safety](https://www.servicenow.com/docs/~_KFpocooubcllAmnc1USA#install-hs-incident-mgmt__table_ix1_bff_gxb).
6. **Opcional:** If necessary, attach an image about the incident or observation using the Attachments section.
7. Select Save.

## Resultado

Based on the selected category, a safety incident or observation is created in the New state.

As a safety manager or agent, you can assign the incident or observation to yourself and start working on it. You can also assign it to another safety agent.
