Record Creator card form
The Record Creator Card form presents elaborate description on the fields that you use to create a record rather than view.
You must have the Desktop Assistant administrator [sn_dex_desktop.admin] role to access the form.
| Field | Description |

| Name | Unique name for the card title. |
| Header | Unique name for individual pages in the card. When you select a card, it takes you to the details page. The name in the Header field appears as the header or title of that detailed page. |

| Application | Scope of the application. |
| Icon | Image that appears on the card. Use the Click to add link and select Choose File to select an image from your system. |
| Table | Table from which you want to pull the data to display. |
