    Before you begin

    Role required: none

    About this task

Each assessment or survey appears as a clickable card in the queue, which can be launched by clicking the card. The card includes helpful information, including the survey name, state, and due date. You must answer every required question, indicated by a red asterisk, before you can submit the survey. If you start to take a survey but can't complete it, save your responses and return to it later. When you've answered all the questions and you're happy with the responses, submit the survey.
    Note:
If you've signed in as a user who doesn't have access to the trigger record, and you access a survey using the URI parameters, the associated record number isn't displayed. You can still take the survey.

    Procedure

    1. Navigate to All > Self-Service > My Assessments & Surveys.
Users with the assessment_admin role, including survey administrators, can display other users' assessments and surveys in addition to their own. Use the Show all and Show assigned to me related links at the bottom of the queue to show and hide assessments and surveys. Click a card assigned to another user to open the associated metric type or survey definition.
    2. Click the survey card to open the questionnaire.
If there is more than one survey category, click the collapse icon (Collapse icon) or expand icon (Expand icon) to hide or show the questions in the category.
    3. Answer each question to the best of your ability.
If you're unsure of how to respond to a question or if a question doesn't apply to a particular record, select Not Applicable, if available.
    4. Complete one of the following steps.
      OptionDescription
Save your responses without submitting them Click Save. You can close the questionnaire and access it later from your queue.
      Note:
The system doesn't save the survey when there are invalid responses, such as a letter in a date field. You must enter valid responses or remove invalid responses before you can save the survey.
Submit the survey after answering all questions Click Submit. You can't return to the questionnaire after submitting.
      Note:
If there's an unanswered required question or an invalid response, an error message appears and the problematic questions are temporarily highlighted.
