Use the mentoring relationship form to start a new relationship between a mentee and a mentor.

| Mentee | Name of the mentee. |
| Mentor | Name of the mentor. |
| Duration | Default duration of the program in months. |
| State | State of the program. |
| Start date | Date on which the relationship begins. |
| End date | Date on which the relationship should end. |
| Closure reason | Option to auto-close mentoring relationship post due-date. |

| Request date | Date on which the relationship is requested. |
| Withdrawal date | Date on which the relationship must be withdrawn. |
| Request note | Details about the relationship request. |
| Decline details ||
| Decline reason | Reason for declining the relationship. |
| Decline date | Date of declination of the relationship. |
| Decline note | Details about why the relationship was declined. |
