Respond to a questionnaire about an application for risk assessment.

## Before you begin

Role required: sn_grc.business_user or sn_grc.businesss_user_lite

## About this task

The Risk Identification form contains multiple stages for the
workflow. When an application is created in GRC as an entity, a
questionnaire is sent to the IT application owner. The questionnaire is useful for
risk managers to identify threats and mitigating controls.

## Procedure

1. Navigate to AllSelf-ServiceMy Assessments \& Surveys.  
   Alternatively, you can also navigate to Risk identification classic questionnaires or Risk identification questionnaires from the My Tasks page to access the questionnaire. Select Risk identification questionnaires for the smart assessment questionnaire.
2. Open the record with the assessment that you must respond to.
3. Enter your responses.
4. Select Submit.

## Result

The state of the risk identification record moves from the Information Gathering state to the Review state.

## What to do next

The IT Risk Manager can review the responses and can either send
the questionnaire back or accept the responses.
