The issue form enables you to create an issue for a demand.

| Field | Description |
|-|-|
| Number | System-generated ID number with a configurable prefix. |
| State | Current state of the issue. Issues are created in the Open state. The available values are: * Pending * Open * Work in Progress * Closed Complete * Closed Incomplete * Closed Skipped {#create-issue-form-ppw__ul_whv_d4k_bvb} |
| Priority | Urgency and importance of the issue based on the possible impact. Issues are created with Low priority. The available values are: * Critical * High * Moderate * Low * Planning {#create-issue-form-ppw__ul_jnw_4fx_tjc} |
| Estimated cost | Estimated cost generated by the issue. |
| Impact | Impact of the issue on the demand. Issues are created with Low impact. The available values are: * High * Medium * Low |
| Due date | Requested date for issue resolution or closure. * If the issue has an assigned resource, the due date is for resolving the issue. * If the issue isn't assigned to any resource, the due date is for closing or addressing the issue. {#create-issue-form-ppw__ul_mxc_kmk_bvb} |
| Assigned to | Resource assigned to work on the issue. |
| Parent | Demand associated with the issue. This field is auto-populated. |
| Short description | Brief summary of the issue. |
| Description | Brief description of the issue and its impacts on the demand. |
| Work notes | Information about the issue. |
