The action form enables you to create an action for a demand.

| Field | Description |
|-|-|
| Number | System-generated number with a configurable prefix. |
| State | Current state of the action. Actions are created in the Open state. The available values are: * Pending * Open * Work in Progress * Closed Complete * Closed Incomplete * Closed Skipped {#action-form-dw__ul_j2x_hdp_c3c} |
| Priority | Urgency for implementing or approving the action based on impact. Actions are created with Low priority. The available values are: * Critical * High * Moderate * Low * Planning {#action-form-dw__ul_ttg_rwx_tjc} |
| Estimated Cost | Estimated cost generated by the action. |
| Impact | Impact of the action on the demand. Actions are created with Low impact. The available values are: * High * Medium * Low {#action-form-dw__ul_jwr_5wx_tjc} |
| Approval | Status of approval from the stakeholders. The available values are: * Not Yet Requested * Requested * Approved * Rejected {#action-form-dw__ul_avr_d2p_c3c} |
| Assigned to | Resource assigned to implement the action. |
| Due date | Requested date for action approval or implementation. |
| Parent | Demand associated with the action. This field is auto-populated. |
| Short description | Brief summary of the action. |
| Description | Brief description of the action and its impacts on the demand. |
| Work notes | Information about the action. |
