Populate Catalog Item from Guided Self-Service with Dependent Category Fields

I am creating a Guided Self-Service playbook in ServiceNow with two questions:
Category and Subcategory (dependent on Category).

After the questions, the playbook opens a Catalog Item.
How can I configure the Subcategory field to show values based on Category
and pass both values to auto-populate the Catalog Item fields?

I Created an activity that includes a question and two fields: Category and Catalog Item.

Once the activity is completed, the Catalog Item form which is selected before should be populated automatically.

The selected Category from the activity should be pre-filled in the Catalog Item form when it opens.
