Approval email notification triggered twice for the requested for user

When an RITM is approved or rejected, email notification is sent twice to the requested_for user.
Send to event creator is enabled and when checked in emails logs. two entries are found which has triggered the email notification to the requested for
Also checked in event logs notification engine has triggered twice as well
Could you please help

Hi @Abhilash ,
I think the record is updated twice in back end and that is the reason for the notification to trigger twice.
Try to change the Condition as Request.Approval "Changes to" Rejected.
I think this condition will solve the problem.

still it doesn't work, this time notification doesn't trigger at all when approval is rejected

any further help on this, i did some changes but notification triggers twice still and one thing i noticed is under audit history logs, i see approval is updated twice to approved, but not sure what is causing the issue
