If we receive a RITM from a customer, is there a way to create a Change Request directly from that RITM?

You'll need to create a new UI Action for this, as there is not anything OOB on RITM to create a Change. Utpal's examples would create the new records.
Only thing ot keep in mind, the examples given will also automatically insert/submit the new records, so keep this in mind if you want any fields on the generated Change Request to be mandatory. They will need to be populated on creation or you will have blank fields that will be left waiting for someone to open the Change and then manually populate them.
