Change Risk assessment is blank

In my sub prod environment I have change risk assessment working correctly. In test environment the pop up risk assessment is blank with no questions. See attachment.

I have changed the change states by adding a new state and re-configuring the order but I cannot get the risk assessment to display the text. As  result all changes come through as Low/Low.

I can see no error messages on the log.

Any thoughts about the resolution to this issue or pointers to help resolution as an added pressure the environment goes live on Friday!

Hi Jhon Rob,

Are you facing the issue only in one environment ? Then try to open the instance in other browser. Try to remove browser cache and ServiceNow cache.

while i was configuring, i can able to see and edit existing risk assessment, when i trying to move dev to test instance, i am getting existing risk assessment blank?

I am having the same issue in my SN Dev instance - tried on multiple browsers.  Did you ever find a solution for this?

Any solution for this?  I'm sure I have something missing or not configured that is causing this, but any insight would be appreciated?
