Create Email template on change record

Need help on to create Email template: Please find the attachment for reference.

*Template is below:

To : <Change owner email >

Subject: check review of changes deployed last week - CHG0XXXXXX

 

Hi <Change owner Name>,

I'm emailing to advise that after completing random compliance checking of changes for last week, we have found that the above change has failed in its compliance due to the following: 

Testing evidence not meeting quality requirements.
Evidence of testing sign off is missing ./Evidence of testing not signed off by appropriate person.

This template will appear on Change Record email function When the  below Change Request conditions are met

Type of change is Normal
State of the Change is closed 
Risk is Low
Process is Prod
Related List conditions 

Approvers =0 
State is approved 
Approval Type is Change management Approval

You can configure email client template for change_request table, check docs link below

Create an email client template 

Ensure Email client is enabled on your table -> ensure email_client=true is present in the attributes in Collection field
