How to add on hover popup message using i icon to select box choices selection for catalog form

Hi All,

i am creating a catalog item in that I have a select box as Request Type(request_type) with choices as

        'working capital inquiry',

        'capital expenditure inquiry (pa)'

        'capital expenditure inquiry (non-pa)'

if 'working capital inquiry', is selected i need hover popup message using i icon to be displayed similarly for   'capital expenditure inquiry (pa)' and 'capital expenditure inquiry (non-pa)' but the texts are different.

Please check the attachment i have to setup as per the screenshot which i attched.

how do I do that? kindly help me.
