Need Export form fields to Word document step by step implemention from below youtube url

Hello,

My requirement is to export the form fields into a word document that i needed (not all the fields) . I have found an intersting implemenation in the below youtube url but i would need the steps for implementing the same.

I need it when we open a record to show a button ui action : download on click of it which will export the form details into msword.
