Probable Cause:
If your flow/workflow contains two consecutive approval steps (for example, Step 1 followed by Step 2), the approval engine may reprocess the full approval block or initiate the second action, unintentionally resetting the first approval status back to Requested.
2. If your approval rule is configured as Anyone approves for a group or user list, approval from a single user may complete the entire activity. As a result, when the flow refreshes or updates, other users’ statuses (or even prior approvers’ statuses) may change to No Longer Required or revert to a pending state.
Probable Solution:
1. Make sure that the approval rule on the second level approval is "Anyone approves or rejects".
2.Add a Flow Logic > Wait for a duration of time step between Test Approver 1 and Test Approver 2
