Standard Change Template issue

Hello Community,

I am working on standard change template, we have approximately 300 templates. our record producer change form is designed like this when we go to raise a request from service portal, when we select a particular template the form will show the values of template like implementation plan, risk and impact analysis those fields.

But now we have issue with 2 (consider X, Y) templates, while I am creating a record from service portal by selecting Template X at that time the form is showing the correct values of selected template(X) implementation plan, risk and impact analysis it is happening before submitting the form. As soon as after form is submitted when we check the change created it is showing the values of template Y.  Rest of the templates are working fine only issue with these particular X, Y templates checked both versions and proposal information also they are different only but somehow this issue is happening.

can somebody help me on this issue.

Better log a Now support case, also before that please check the Flow as well may be template name wrong there or used.
