Unable to set values in Risk Assessment form in change request ATF

For risk assessment the required values are not popping in execution .I tried adding script . Is there any other possible way to solve this?

Can anyone help me with this. It will be great help.

There is no way, it is something a known issue and this is as expected bahviour.
