WorkFlow not attached to RITM (not all the time, its occurring sometimes 1-2% chances)

Hi Team,

 

I have catalog item (General IT request) and it has Workflow.

From Incident form we have UI Action to create Catalog Request (using script).

98% requests created from UI action are having workflow attached.

1-2% requests Workflow are not being attached to the requests.

 

what could be the case ?? what I need to check (as it's not occurring for all the cases, some random cases).
