Many to Many on Same Table (Problem Table)

We have a requirement to create a many to many related list in the problem record . For example if you add a problem record in the related list of current form then the current problem record should be shown in the related list of the problem record which you have added here and also you should be able to see the edit button and able to add any number of problem records.

Please help asap.
