VR Request Extension not working

I am testing our approval rules for VR and I am to the point where I am testing an extension of an exception. When I click the UI action to request an extension the Window opens for me to enter the date and reason but after I submit that nothing happens. No approval is sent, no questionnaire pops up, the dates of the exception do not update.

I have setup approval rules for "Exception Rule Deferral Extension". That is set to assign using = approval table field, Approver table = Exception Rule and Approval table field = Exception requested by manager. The user requesting has a manager in the manager field with needed approval roles (exception, false positive, unassign). I have tried updating the rule to use a group, a user but nothing seems to change. 

 

Is the extension supposed to have a questionnaire if the exception has one? Is there something I am missing for the request extension to work?

Gotcha, another thing I noticed in your Approval Rule screenshot; there is no approval configuration related list. Here is a filter on sn_sec_cmn_approval_config for the OOB extension for exception rule. Is yours active for State Approved? See screenshot below.

The approval flow is run through Flow Designer, so you might be able to check the flow execution logs to find any errors when requesting extension from exception rule.

That record is active for that condition. I will check flow logs to see if anything pops up.

Not seeing anything trigger in the flow operations logs either.

Were you able to solve the issue?  currently I am facing similar issue with Exception request. It suddenly stopped working. 

I think my issue may have been due to some updates we made to one of the questionnaire's. Once we reverted that back to OOB the extensions were working. Will check my Hi cases though.
