Existing Operational Resource Assignment has reverted to Pending and unable to Approve

If I change the Resource Status to Approved and click Update or Save it flashes and reverts back to Pending without any error message.

We have discovered that the Operational Resource Assignment Resource Status changed from 'Approved' to 'Pending' when a time card was submitted.  It is now locked in Pending status and we cannot change it back to Approved.
