Field Update Cause Duplication in Activity Stream & Affect Email Notification

Hi Experts!
Kindly seeking Community advice regarding an issue with the Risk Event form. When a field value is updated, the Activity Stream sometimes logs the same field change twice, showing two identical lines for a single update.
This duplication also impacts our email notifications, which are configured to trigger on field updates. As a result, two identical emails are sent for what should be a single update.
What’s particularly inconsistent is that:
- Sometimes, the same field update appears only once in the Activity Stream (as expected)
- Other times, it appears twice, even though the field was only updated once (This is the issue)
This behavior is unpredictable and seems to vary over time.
