How to implement Minutes of Meeting concept

Hi Everyone,

I am having a requirement, suppose in a incident, RITM, Problem etc. When we open any incident form in the related list of incident I want a related list name--"minutes of meeting" in which we can provide the timing of the captured meeting which was done against the incident. And also if the time is in future then a email should be triggered to all the recipient.
