We are trying to create a user input form with the ServiceNow UI. The web based form will allow the general public to input data and pay fees. I realize that there is some confusion as to what constitutes a FORM on the ServiceNow platform. What we need is a "simple" form similar to what you would see on Google Forms for example. Please see attached. We have had considerable difficulties using the UI to generate input boxes that allow the user to save data to a table. The component "widgets" that we have tried offer limited utility at best, we cant bind data to the control.
