How Do I Create A Loaner Laptop Form in Service Catalog

We have a small Service Catalog that now contains Create Incident, Order IS Equipment, Media Equipment Services Request and SAC (System Access Control). I want to build a new form for the first time on my own for Requesting a Loaner Laptop
