Add Incident details in Survey form in ServiceNow

Incident number should be show dynamically while we open the survey link for providing the survey.
which help to user to know the incident details for which he is providing survey.
Note : Dont want to use dropdown selection for displaying incident number.

Thank you, after following these steps incident number is not auto populated it is showing blank only.
but how the survey link will be trigger?
need guidance on this

To survey link be trigger you need create a trigger condition
