Adding fields to the Approval Record Form on Service Portal

I need to add more information to the Approval Record in the my approvals area in the Service Portal. Right now the user can go to the My Approvals page and see a link to the approval that is needed, but when they click it and drill in to it they get a very abbreviated version of it and there isn't much info on there for what they are really approving. To make life easier for the approvers I've been asked to show more information.

Currently if you go to My Approvals from the Service Portal you get this:

If I click on the approval for the KB listed it takes me to the Approval Form page that looks like this currently:

What the we would like to have is something like this in addition to what is shown above:

I have cloned the widget that is related to this page and tried to add some fields in to get them to show, but I am not having any success with that. Is there a way to get all of the requested items information on to the Approval Form portal page?
