Adding Watch List to Custom Form in the Service Catalog

I created the following form in the Service Catalog.

I have a request to add the "Watch List" icon as an option for our end-users when submitting any requests for service.  (diagram 1).  This view is what our end-users see when submitting initial request.

I was able to include the "Watch List" option on the REQUEST and RITM form (diagram 2 and 3), but this is accessible by ITIL Users after the Request has been created. 

Question:  How can I add the "Watch List" icon on the Custom Form?

Hello,

 

usually, I set a slushbucket in the catalog form as it will pass a list of user's sys_id in string separated by a , and the watchlist is this format too.

On submit I populate the watchlist with the slushbucket value.

 If you want the same control as the watch list I think it would require an UI macro (if someone could confirm that)
