Is there any way to modify the service catalog's "ask for approval" to include different fields instead of the default ones?

Absolutely, if you go with create approval record you can include different fields on approval record and proceed with wait for approval condition.

@RampriyaS but how do you do that? I am not experiensed with ServiceNow just learning so any advise is appreciated.
