is the approver active ? Is the flow set to "RUN AS SYSTEM USER" ? 

Is the group active - if group approval?

Did you check approval record got inserted into sysapproval_approver table for that RITM?

if yes then OOTB notification on sysapproval_approver table should trigger the email

Are you sure that approver is?

1) active and not locked out

2) has email address populated in user record

3) has notification preference enabled

Are you sure the approval really triggered?

Share the flow or workflow using which you are setting the approval user/group

Above record is as it is Still the Approval Mail did not got triggered.
