Breadcrumbs not working Service Portal

Hello all,
The breadcrumbs that show the navigation path within the Service Portal are not working for all parts apart from the Knowledge Base section in our instance.
When using the Knowledge Base section and navigating, the breadcrumbs show as below:
Whereas when using another section which navigates through multiple Icon Link's, it displays as below after three Icon Link's:
The Knowledge Base section only uses the one Icon Link before it goes into a KB Categories widget
Does anything need to be configured to resolve this?

Several questions:
1. What's the page that has the accounts & access breadcrumbs? Is this within a service catalog page or something else?
2. Can you ctrl+right-click the breadcrumbs and then choose "page in page designer" and please take a screenshot as shown below:

Hello JaeggerLegane,
The route through to this page is as follows:
Icon Link called 'IT Services' > Icon Link called 'Request' > Icon Link called 'Accounts & Access' and within the A&A page, there are URL Icon Link's through to catalog items.
Here's the screenshot, it just has Home >. This is the same for all of the Icon Link's through to Accounts & Access...
