Customise 'Create Incident' page in Service Portal

Hello,
I've been playing around with a trial instance of the new Helsinki portal and I cannot workout how to edit a page like 'Create incident' [Home>Service Catalog > Can we help you>Create Incident]. I looked at Page editor and widget editor but this page is not listed. Basically, in my workplace, we use more form fields when creating incidents as we use the ServiceNow admin to do this job. However, with the introduction of service portal we would like to introduce the portal side for non-IT users but we still should be able to create something like the attached mock-up as the form and wondered if this was possible to achieve?
Basically, I'm trying to create the exact form in the admin site in Service portal.
Any help is massively appreciated.

Could you please provide the screen shot of content item that you created as it is still opening in a new tab ever after making all the settings as you said.

Are you clicking from the desktop or the Service Portal view? Below is for the desktop, Service Portal will only work if you use the portal page direction to a created page for incident submission. We don't have that as we use a record producer instead of the default incident form, but it should be able to be done as long as you have created the page and widget(s) to support the incident form.

Thank you all for your replies! I've only been using ServiceNow admin site to raise requests, incidents etc but never have involved in developing anything within the system so all these terms sounds a bit 'alien' to me but I will read the wiki and see if I can make sense of them.
