Customize Approval Form 'Summary of Item' to display End-User Filled Fields for one Catalog Item

We need to modify "Summary of Item being approved" in  the approval form so that it dynamically displays the values entered by the end user for a particular catalog item.
Currently, the Summary of Item being approved shows all the variables of catalog item 
The UI Macro used :approval_variable_summary

Here is correct explanation of issue

 

We need to modify "Summary of Item being approved" in  the approval form so that it dynamically displays the variables which has values entered by the end user for a particular catalog item. We don't want the field to be displayed if the end user has not entered any data/ check for the checkbox 
Currently, the Summary of Item being approved shows all the variables of catalog item even variables don't have data and checkbox is unchecked
