Data visible on view but not form

Hello All,
There were few changes made to the domain parent-child relationship on sub instance for the data to be visible in performance analytics. We are in a testing phase to understand what are the impacts it may had on existing functionality. I came across an issue (not sure if this is related to domain modification) - whenever an engineer tries to create an incident (fills in the service, service offering, assignment group and assigned to field), the incident gets created but the mentioned field data gets deleted automatically once I submit/ save the form. Like below:
This is happening with fields:
-Service offering
-Service
-Assigned to
-Assignment group
-Resolved by
-Deparment ID
-Cost Center ID
I can see the same incident data in list view as below:
I am unsure where I need to debug, would this be domain issue or ?

Please check the Display value set true for the name on the table.
