Empty values on Approval notification

We encountered an issue where an approver received an approval notification with all values missing. I reviewed the event logs and the associated business rule, but everything appears to be functioning as expected.
The approval notification uses an email template, and this is the first time we've experienced such an issue, which is puzzling.
Please refer to the attached screenshots comparing the empty notification with a correctly populated one.
