why are you using Create record? what's the table you mentioned in Create Record step?
if you don't give table name then it will throw that error
Simply use custom flow action and call it and pass the RITM sysId and then update there

this API SNC.AssessmentCreation() is at platform level and we don't know how it creates the assessment instance record
