How to create a flow designer for below attachment and explain step by step to create and at last how to check in the form after completion of flowdesigner. Please use a incident table to trigger the attached flow.

firstly, create a new flow, define a table and decide whether it is insert or insert and update, then continue step by step as per the diagram.

I do not understand what is the issue. Please, could you share screenshot to understand what is the problem?

Hi, it's not an issue. Iam asking how to create step step and what we have to mention in that trigger conditions and actions.
