Flow execution failing

Hi All, Please help with the Flow designer issue. Flow execution is failing to start flow once the item is submitted. It is not able to recognize related request numbers. Tried the Wait condition action, but still getting the issue.

Thanks for your response, But this is not an issue with linking flow and catalog items. Flow starting is delayed. Please see the attached snapshot.
