Flow is not working properly

Hi ,

I have created custom form and each and every state will change automatically based on approval like

InProgress->working->test->solve

each and every step have approval user when everyone approved it will  change state automatically .

but flow i have created properly and its correct but its not working on random records how to find this

On which table you have created flow ...

This is custom table  it will change the each and every state based on approval from that state but it not working for random records
