Form Page is missing from PDI

Hi

I am unable to display any records from any tables because the 'form' page itself is not available in PDI.

So id=form&table=incident always results in a 404.

Also when setting up a simple list widget the list page and behaviour dropdowns do not contain 'form' as an option. 

How can this be fixed?

Please release this instance and take a new one. Its PDI and if you get new PDI , then it may not get upgraded successfully.

This sounds a bit drastic Atulya. I suppose I will lose all my work.

Can I create a new page called form with a form widget in it?

I am not sure, but give a try to create a new page. The reason I am worried, these are OOTB configuration and we dont know where these names/ files/ component used in what sense. May be creation will solve problem, or not.

Also before releasing , take a back up of your work and put in to new instance.
